FORMS

All of the forms listed here are available at Bing's Boarding and you can complete the required forms when you come in. If you'd like to have them completed prior to, you can access the forms here to fill out at your convenience. If you choose to do that, please make sure to print them out and bring the completed forms with you or you can download and email the completed forms to us. 
Click on the form below you would like to complete. 
Registration Form- All clients (boarding, dayboarding, grooming) will need to complete a registration form. Please fill out one registration form for each dog.        **Registration forms need to be updated on a yearly basis. 
Quick Boarding Check-in Form (Return Visits)- All returning boarding clients will need to complete a quick boarding check-in form for each visit.    If there are ANY changes to Meals or Medical Needs, you will need to fill out a full boarding check-in form to update any changes. 
Boarding Check-in Form (First Visit)- All boarding clients will need to complete a boarding check-in form for their first visit. 
Grooming Registration Form- All new grooming clients will need to complete a grooming registration form. 
Medication Form- All clients requiring medication and/or supplement administration will need to complete a medication form for each visit.
Dog Training Agreement Form- All clients enrolled in a Bing's Boarding dog training class are required to complete the Dog Training Agreement form.
